Book A Vaccination Appointment with Phone

To book you must:

• • meet the current eligibility criteria as outlined in phase one and phase two of the Ontario three-phase plan (Please Click HERE to check your Eligibility)
  • have a green photo health (OHIP) card
    
  • have an email address (or you can use the email of the person helping you)
  • provide your phone number to contact you about your appointment

How we use your information

We will only use your personal health information to:

• • confirm it’s your turn to get a vaccination
  • schedule and confirm your vaccination appointments

We will only use or disclose your information if we are legally required or permitted to do so.
